Chronic Disease Self-Managment Programs help those with conditions such as Diabetes, COPD, Asthma, Arthritis, Multiple Sclerosis, and many other conditions, including chronic pain, learn effective self-management skills to improve their health.
Learn how to manage day to day health issues. Over the course of 6 weeks you will learn practical ways to handle topics such as:
- Medication use
- Healthy eating
- Physical activity and rest
- Dealing with fear, anger and frustration
- Communication with family and health care providers
Programs in this series include Healthier Living Workshops, Chronic Pain Self-Management and Diabetes Self-Management. These programs offered by Community Education and The Community Health Team of Rutland Regional Medical Center and partially funded through the Vermont Blueprint for Health.
