PROJECT DESCRIPTION The Behavioral Health Clinic (BHC) will serve a vital community need for outpatient behavioral health services by providing a wide array of services to the adult patients from age 18 to geriatric care. The clinical services offered by the BHC will be provided by a multi-disciplinary team of professional staff including: Psychiatrists, Psychologists, Advanced Practice Nurses, and Social Workers. The professionals will work closely with patients’ primary care and specialty care physicians to establish collaborative treatment relationships.
All new patients will be referred by their physician through the use of the BHC referral form. All psychiatric consultations require a physician referral. Although self-referrals will be accepted for counseling services, clinic patient who need a psychiatric evaluation will still require a referral from their primary care provider. In most cases, it will be the expectation that the primary care provider remain actively involved in the ongoing patient care. It will be a program expectation that all patients provide the necessary releases of information to adequately coordinate patient care with the PCP and other health providers involved in the patient’s care.
LOCATION The clinic is located on the 5th Floor of Rutland Regional Medical Center at 160 Allen Street.
CLINIC HOURS Monday -Thursday 8:30am to 5pm Friday 8:30am to 2:30pm
SCOPE OF SERVICES Outpatient Consultation Program Psychiatric consultation is offered to adult patients (18+ years of age) receiving care at participating primary and specialty care clinics within the Rutland region. Services are provided on a consultation basis with the primary care physician retaining responsibility for all follow-up care and prescriptions. After a comprehensive evaluation of the referred patient, a written evaluation will be provided to the referring physician within 3 business days. Following the initial consultation the consulting provider, the primary care physician and the patient can jointly review the need, if any, for ongoing psychiatric care by clinic providers.

Outpatient Counseling Program Outpatient psychotherapy services are offered to adult patients to address a variety of emotional and behavioral problems. The patient and therapist will work collaboratively to develop treatment goals and plans. Agreed upon treatment modalities may include individual, group or family therapy as indicated by the identified issues and patient preference. The program emphasizes a brief psychotherapy /solution focused model of treatment in which patients identify and work toward resolution of targeted issues. Since not all patients or issues are appropriate for this type of treatment, an initial comprehensive evaluation will be conducted for patients entering the program. Patients for whom the recommended treatment fits within the program parameters will work with the therapist to define a plan of care. Other patients whose needs are beyond the scope of the program will work with the therapist to obtain appropriate referrals.

APPOINTMENTS AND REFERRALS
Questions regarding new patient referrals and insurance coverage may be addressed by calling our clinic. To make a referral, the referring provider must complete the BHC referral form and fax it to 802.747.0129. Referrals received before 4pm will be reviewed and referring providers will be contacted by the end of the next business day.
