Physician Referral

If you’re interested in seeking treatment at the Rutland Diabetes & Endocrinology Center, you will need a referral from your physician or nurse practitioner. The reason for this is to be sure your provider is involved from the start. Please call his/her office to learn which forms and information are required for referral within your insurance plan.

Insurance Coverage

Most insurance companies cover the cost of diagnosis and treatment of diabetes and endocrine disorders. Many also pay for all or a portion of diabetes education services. Please check with your insurance provider to learn about your coverage and find out about the co-payments and referral requirements for your particular medical situation. Our staff is familiar with the requirements of many insurance companies and can help with your insurance coverage questions.

Appointments

Appointments with the Rutland Diabetes & Endocrinology Center care team are available Monday through Friday, 8am-4:30pm. To schedule an appointment that suits your schedule, please contact our office at 802.775.7844. We request that you kindly provide at least 24 hours notice in the event you must cancel or reschedule an appointment.

Free Diabetes Prevention Classes 

Our diabetes educators offer free monthly classes for people who are at risk for developing diabetes. Are you at risk? Take a risk assessment and get more information on what to do to lower your risk.
