Becoming a Patient

Anyone in need of these services can self-refer by calling 802.776.5800. The intake process includes a full biological, psychological and social assessment to determine best treatment planning. This includes:

  • Initial Phone Screening
  • Physical, including Urine Screens and Blood Work
  • Psychosocial History and Psychological Evaluation
  • Medical Assessment 

When the screening process has been completed, a daily dosing schedule and treatment plan is established. 

Eligibility Requirements:

  • At Least 18 Years of Age
  • Have a Photo ID
  • Evidence of Opiate Addiction
  • Agree to Adhere to Program Policies and Procedures

Payment for Addiction Treatment

The Center accepts all types of insurance. Grant funding for treatment services is available for qualified patients.
