Rutland Regional Medical Center joined OneCare Vermont because we believe in offering the best care possible to Vermonters, and because we support the shift to value-based care.

OneCare is Vermont’s local, 501(c)(3) Accountable Care Organization (ACO) that partners with healthcare providers to transform the state’s healthcare system. OneCare doesn’t provide healthcare – instead, it brings health groups together to improve healthcare quality and stabilize costs. By managing network performance, giving healthcare providers data and analytics, and paying for quality of care and improved population health, OneCare supports organizations like Rutland Regional in our work to deliver excellent health care to Vermonters.

What Does OneCare Do?

OneCare helps improve healthcare for Vermonters. Healthcare providers in OneCare coordinate across organizations and share resources and data. They focus on connecting patients to primary care, coordinating care for patients, and managing chronic illness. The goal is to help Vermonters become healthier — not just care for them when they are sick.

You, your doctor, and your insurance company work together to ensure you receive the healthcare you need. Your health benefits and coverage stay the same, and OneCare does not limit healthcare providers or number of visits.

Healthcare costs are too high. Vermont is leading an effort to change the way healthcare is paid for or incentivized: value over volume. OneCare works with insurance companies to reward providers for wellness, not illness. Instead of paying for each test and procedure, OneCare pays doctors a flat amount to care for patients. Over time, these changes should lower healthcare costs for Vermont.

OneCare gives doctors data, resources, and tools to improve patient health. By sharing information and working together, providers can do even more to give the best care to Vermonters.
