Do your loved-ones know how you would like to be cared for if you couldn’t make those decisions for yourself? Planning your Advance Directive is a sensitive subject, but it is something that all adults, young and old, should be thinking about. Give your loved-ones the peace of mind that comes from understanding your wishes before a crisis occurs. In recognition of National Healthcare Decisions Day, please join Rutland Regional Medical Center and the Community Heath Team for a discussion on this very important topic. Eva Zivitz of Rutland Regional’s Palliative Care Program will review the forms used in Vermont to document health care wishes, and will walk participants through the process of completing them. Volunteer explainers from Community Health Team will be available to answer individual questions or assist those who are interested in making an appointment for assistance in completing an advance directive at a later date.
This event will take place on National Healthcare Decision Day, Tuesday, April 16, 2018.
Refreshments will be provided.
