The Community Health Improvement team is dedicated to improving the overall health and wellbeing of our community.

It’s Community Health Improvement Week and we’d like to celebrate and highlight the Community Health Improvement Department (CHI) at Rutland Regional Medical Center (RRMC) and the highly trained staff who provide support to community members with challenging and sometimes complex needs.

The CHI team is dedicated to improving the overall health and wellbeing of individuals through collaboration and community partnership and work with a number of programs and initiatives that support people in achieving their goals.

One such program is the Blueprint of Vermont initiative. Through this program, the CHI team provides care coordination, patient advocacy, and the navigation of health care and community supports while partnering with Primary Care Providers. The team helps patients and families of all ages get access to the needed services and supports to help manage health concerns and access community resources by identifying challenges that interfere with efforts to better manage their overall wellbeing. These challenges can be things like accessing specialty providers, life stressors such as financial barriers to access care, or needing condition-specific wellness education. Community Health Team Care Coordination services can be accessed within Rutland Regional Medical Center as well as through Blueprint for Health Primary Care Practices.

Self-Management Workshops are another way the CHI team supports the community. These workshops teach ways individuals can better manage their own health, and they are free. The workshops are designed to support managing chronic conditions like diabetes, hypertension, and chronic pain through peer facilitation and support as well as evidenced-based education. Workshops occur in person as well as online with multiple days and times.

The Lifeline Program is another service that supports our community through its medical alert system. The ability to have access to an emergency response system supports independence and offers reassurance for individuals and families.
