As Vermont’s second largest hospital, and largest community hospital, Rutland Regional Medical Center has a key leadership role in implementing the Vermont Blueprint for Health for the hospital’s service area. The Community Health Team (CHT), an integral part of the Blueprint, is coordinated and supported by Rutland Regional and is actively working with patients in Blueprint practices. The CHT goal is to coordinate care across multiple health related organizations to help patients at home, in the doctor’s office, and during transition between care sites.

A process to identify and then address patient care needs within a primary care practice fulfilled by panel managers.
Targeted toward reducing readmissions of all patients, this program will start by focusing on reducing the number of patients discharged with a diagnosis of CHF from being readmitted within 30 days. The program will also be applied to other chronic conditions.
Making sure the needs of the patient are identified and met through a care coordinator that supports their primary care physician.
Individualized or group support that leads to improved self-management of any disease process by the patient. This includes referral to existing resources such as Healthier Living Workshops, Bone Builders, Tobacco Treatment, etc.
Working with government and community partners, such as Partners for Prevention, to assure that our communities encourage bike and walking paths, nutritious food choices at the corner market and smoke-free venues to name a few items.
Rutland Regional Medical Center’s Self-Management Programs provide participants with the skills, tools, and information they need to manage their medical condition. Our goal is to help them lead happier healthier lives. These programs provide evidence-based guidance, education, and information to assist people in coping with a variety of chronic conditions, including:
All Self-Management Programs are free of charge and funded in part by My Healthy Vermont. Our programs are delivered in a group workshop format and facilitated by trained leaders who have a personal understanding of chronic disease.

Chronic Disease Workshops provide general chronic disease management information and guidance designed for individuals who are living with a variety of conditions, including heart disease, diabetes, depression, liver disease, bipolar disorder, emphysema and more.
The workshop meets for 2.5 hours once a week for six weeks.
Chronic Pain Workshops will help guide and support you in learning to manage common problems that can come with chronic pain—like fatigue (feeling tired), trouble sleeping and being socially isolated, among many other things.
This workshop meets for 2.5 hours, one day each week for a total of 6 weeks.
Our small-group Diabetes Management workshops are led by specially-trained professionals. Vermonters who’ve completed the program say they’ve seen big improvements—like eating better, feeling less depressed, and having a better relationship with their healthcare team.
The Diabetes Prevention Workshop (also known as Prevent T2) is a proven program to help you make small lifestyle changes. You can cut your risk of type 2 diabetes by more than half. It is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC).
This workshop lasts for one year. There are 16 weekly core sessions, plus monthly maintenance sessions. Each session is 1 hour long.
The High Blood Pressure Management Workshop (also known as Health Coaches for Hypertension Control) is a proven, small group program developed to help participants ages 18 and older take control of managing their high blood pressure at home.
This workshop meets weekly for 1.5 hours, for 8 weeks.
Our local Vermont Quit Partners offer FREE, weekly, small-group workshops across the state. Your workshop leaders and fellow participants will guide and support you to create a plan of action for becoming tobacco free, stay motivated for the long run and find ways to manage your stress and cravings.

Our Wellness Recovery Action Plan (or WRAP®) workshop is a self-designed prevention and wellness process that you can use to get well, stay well, and build the life you want. The program was created by people who successfully overcame their own mental health challenges. All of our specially trained workshop leaders have developed their own WRAP®—so they know what it feels like to go through the process. The program offers 24 hours of content delivered in a series of workshops.
