End-of-Day Vaccine Standby List

Update: Friday, April 16, 2021

  • The End-of-Day Vaccine Standby List is closed and will reopen on Monday, April 19 at 9am.
  • If you submitted information on this form between Monday, April 12 and Friday, April 16, you may still receive a call over the weekend.
  • If you haven't received a call by Monday, April 19, you will need to submit your information again to be placed back on the list for the week of April 19 - April 25.

Please do NOT submit this form more than once per week. Thank you.
