Screening Questions for Continuing 1a Vaccinations –
Under 18

  • The Vermont Department of Health has released new information on getting Phase 1A eligible healthcare workers and first responders vaccinate for COVID-19.
  • If you believe you qualify for the Phase 1a group and have not yet been vaccinated or were unable to receive your second dose and need to get it scheduled, please complete the screening questions below.
