At Rutland Regional, we are committed to making it easy and secure for you to access your medical records. Whether you’re a patient, provider, or authorized representative, there are several convenient ways to request copies of your health information.

Request Your Records Online

Requesting your records through Rutland Regional has never been easier! We have now partnered with Verisma – an industry leader in Health Information Management to allow for safe, secure medical record requests online.

You can submit your request for medical records through Verisma’s secure online portal. It will guide you every step of the way and all you’ll need is access to a camera or smartphone to upload a photo ID.

The typical turnaround time for requests that meet all HIPAA and state requirements is generally within 10 business days after completion of the record request.

RRMC Portal

Most of your health information can be viewed online through the RRMC Patient Portal. If you are 18 years or older, you can self-enroll to access your records securely. Start the self-enrollment process now:

NOTE: If you experience any issues with self-enrollment, please call or visit Outpatient Registration. Outpatient Registration is located on the main floor of Rutland Regional Medical Center and can be reached at 802.747.1755.

Request by Other Methods

To request by mail, fax, email or phone, complete the Authorization for Disclosure of Health Information Form (pdf) and send it to Health Information Management via mail, fax, or email. Please include a copy of a valid photo ID.

Records Request by a Physician

Your doctor can request your records.

  • The doctor's office can call 802.747.3654, option 2 to request your medical records. We will fax the requested records directly to the clinic. This is the quickest way for your doctor to receive your medical records.

The Health Information Management (HIM) department will respond to requests for medical records within 30 days.

  • Records cannot be provided immediately or "on the spot" due to the time it takes to gather the requested information and process the request.
  • Medical records may be mailed to you, or you may make an appointment to pick them up.
  • Medical records may be faxed to your physician.

If you are at Rutland Regional...

  • You may visit Financial Counseling on the first floor of the hospital to fill out the authorization form. Financial Counseling is open to the public Monday through Friday, 8am to 4pm.

Costs

There is no charge for copies of your medical record requested from and sent directly to other healthcare providers for further medical care. If the copies are needed for an upcoming doctor's appointment, please note the date of the appointment on the request.

Due to the strict, procedural, and highly regulated steps involved in the release of information processes, requests directed by the patient may be charged a fee. Learn more about applicable fees for patients requesting medical records.

  • The first 10 pages of your printed or electronic records are free
  • A standard fee of $0.50 per page will be charged for additional pages.
  • If you request your records on a CD or thumb drive, a $6.50 flat fee will apply

NOTE: Electronic copies delivered securely through Verisma’s online portal are free.

Attorneys and Insurers requesting records will be billed for the copies according to the number of pages in the record being requested.

Access to Radiology Images

Rutland Regional has partnered with Nuance PowerShare, an easy-to-use medical imaging solution that gives you secure access to view and share your diagnostic images. This secure cloud-based image sharing application provides you and your doctors with convenient access to your medical images and documents across multiple healthcare organizations.

Need a correction made to your medical records?

If you've found an error or need information changed in your medical records, please fill out the Request for Amendment/Correction of Health Information Form (pdf). Once you've completed the form, please mail it to:
